Our department has used a survey in the past and recently revised it. Here are the questions we are using now. Hope you find some of it helpful. I am interested in what other people are using as well.

I understand the interpreter’s signs and meaning.
The interpreter makes fingerspelling and numbers clear.

The interpreter understands me when I sign.
The interpreter voices for me correctly.
Interpreter is professional in her (his) role and work.

Sorry I wasn't more clear. I am hoping to survey interpreters about their experiences working at a the community college where I coordinate--what is important to them and how they believe the college is meeting their needs and expectations.
